Chronic Kidney Disease

Chronic Kidney Disease (CKD): learn how to self-manage this often symptom-free condition through nutrition, diet, medication and regular monitoring through periodic physician visits and blood tests.

The hurdles that this program addresses are:
  • Do you know what stage of kidney disease you have?(Determined by glomerular filtration rate)
  • Do you understand your special dietary/fluid intake requirements and/or restrictions?
  • Are you compliant with your medications?
  • Blood pressure ≤ 130/80
  • LDL ≤ 100

CKD-related office co-payment reimbursement is available when you reach all of the hurdles of this program. After 6 months of uninterrupted compliance with your treatment plan and you could be eligible for CKD prescription co-payment reimbursements, too.
