CHN Disease Self Management Programs

Migraine:  Learn how to keep a headache diary to help identify and avoid your specific triggers and recognizing early signs of a migraine.   Learn the difference between prophylactic and abortive medications and which kinds you should talk to your doctor about.   

The hurdles that this program addresses are:

  • Do you know what type of migraine headaches you get?
  • Do you know what triggers your migraine headaches?
  • Do you have a migraine diary?
  • Do you have/need a prescription for an abortive headache medicine and a prophylactic headache medicine?

Migraine-related office co-payment reimbursement is available when you reach all of the hurdles of this program.
