CHN Disease Self Management Programs
Asthma (for adults and children): Learn how to identify and avoid personal asthma triggers and how to self-manage your condition using an asthma action plan. Children can receive a free computer game that teaches valuable information in a format designed just for them.
The hurdles that this program addresses are:
- Knowing asthma triggers and how to anticipate them.
- Having a peak flow meter and knowing how to use it.
- Obtaining a prescription for a daily long-term control medicine and a quick-relief medicine.
- Having a written Asthma Action Plan.
Asthma-related office co-payment reimbursement is available when you reach all of the hurdles of this program.
